Mailable, Faxable, E-Mailable Order Form

The good old way:
Write everything on a piece of paper and fax it, mail it, or just call us. Our toll free number for orders is:

Please use this number for orders only and from telephones in the Continental United States only. All inquiries other than orders should be directed to: 1-270-777-0744 We will need to know:
- The name and telephone number of the person placing the order
- Your shipping address
- The name of the products you want to order
- How many of each item
- Each item's price
- Payment information including:
- Your credit card number
- The expiration date
- The name of the card holder as it appears on the card
The "low-tech" but very secure:
Print the order form after you have filled it in (without submitting it) and fax or mail it to us.
The "high-tech/low-tech" solution to security:

Fill in the order form with all information EXCEPT the expiration date of your credit card. Send it directly to our office. Then, follow the form with an E-mail message giving us your your credit card expiration date and including your telephone number (with area code) for reference.
